Progress in reducing rehospitalization rates has been slow nationwide, and performance on some measures has actually grown worse. It’s crucial to address hospital readmission rates to avoid poor patient outcomes and negative impacts on your agency’s bottom line—and the quality assurance and performance improvement (QAPI) process is the perfect place to start.

In this 90-minute webinar, expert speaker Diane Link, RN, MHA, will cover how to address rehospitalizations using a QAPI process as outlined in the home health Conditions of Participation (CoP). Jump-start your performance improvement process by identifying and implementing strategies to address this high-risk issue.

At the conclusion of this program, participants will be able to:

  • Understand the basic principles and application of the QAPI model outlined in the home health CoPs
  • Identify and use the data sources required to capture their agency’s root causes of readmissions
  • Use their data in a Plan-Do-Study-Act (PDSA) methodology to create, implement, and monitor a QAPI plan and projects to improve hospital readmission rates

  • Why reducing rehospitalizations is so crucial for home health agencies
  • Types and sources of data to include and analyze as part of a performance improvement project on rehospitalizations
  • Strategies to identify the root cause of rehospitalizations
