New utilization limits, new coding rules, and new imaging guidelines. These are just three of the changes that Medicare Administrative Contractors (MAC) plan to implement for diagnostic and therapeutic epidural blocks. That’s right: On top of the new uniform policy for facet joint interventions, pain management specialists who treat chronic pain are facing a new uniform policy for translaminar, transforaminal, and caudal epidurals. Members of the practice including coders, clinicians, and schedulers must understand what MACs are planning to prevent a wave of revenue-draining denials.

Note: This update does not apply to epidurals performed for postop pain management or labor.

At the conclusion of this program, participants will be able to:

  • Prepare for the new uniform policy and its impact on scheduling, documentation, and coding
  • Prevent errors that will trigger denials of translaminar, transforaminal, and caudal epidurals for the treatment of chronic pain
  • Use the 28 definitions in the policy to improve documentation and coding
  • Explain the different methods for epidural blocks

  • Epidural overview—anatomy and basic coding guidelines for translaminar, transforaminal, and caudal blocks
  • Uniform LCD and article, “Epidural Procedures for Pain Management”:
    • Frequency limits and procedure intervals
    • Steroid dosing limits
    • Imaging requirements
    • New definitions
    • Modifier -KX: when to use it and why
    • New covered ICD-10-CM codes
  • Q&A
